You will need both forms above to register a team. These forms can be used for any adult sports league. Indicate which sports league when completing the form. Minimum and maximum number of players for each sport is indicated on the form. -- We recommend printing on 8.5 x 14, legal-size paper.

Paducah Parks Services Behavior Policy

Paducah Parks Services under no circumstance will tolerate verbal and/or physical abuse of any staff member, umpire/referee, or program participant.  The penalties for any of the above behaviors are strict and strongly enforced.  First time offenders may be suspended for a minimum of 1-year from all Parks Services Programming.  It is our mission that all participants as well as employees feel safe and are protected from any type of abuse.
